Modoc County is one of California's least populous counties (U.S. Census Bureau, 2025), with approximately 9,600 residents spread across 3,918 square miles (SimpleMapLab, n.d.). Its only hospital, Modoc Medical Center in Alturas, is a small critical access facility with no inpatient psychiatric unit, so the nearest full behavioral health hospitalization is roughly 150 miles away in Redding. Behavioral health services in Modoc County are almost entirely absent, with fewer than five licensed mental health clinicians serving the entire county.

The Native American community in Modoc County, including members of the Pit River Tribe, faces compounding disadvantages in mental health access, alongside historical trauma, geographic isolation, and cultural barriers to traditional Western mental health services. Why Telehealth Works Here

Modoc County occupies California's far northeastern corner, and it is one of the most remote places in the state by any measure. Alturas is the only incorporated city, the county's one hospital has no inpatient psychiatric unit, and the practical answer to most specialty care has always been a very long drive toward Redding, Klamath Falls, or Reno. Ranching and agriculture define the economy, distances between neighbors are measured in miles rather than blocks, and winter isolates the county further. Telephone sessions carry equal weight to video here, since connectivity outside Alturas varies, and both are covered. Evening and weekend appointments respect ranch schedules that start before dawn. There is no referral, no waitlist, and no cost for covered members. In a county where the provider shortage is near total, PMHS responds to every inquiry within 12 to 24 hours and typically schedules the intake session within the same week.

Your First Session

After you reach out, your clinician contacts you directly, always starting with a text, then by phone or email depending on what you prefer, to confirm you are ready to get started. You will then get an email invitation from SimplePractice, our patient portal, to finish setting up your account and sign your consent forms electronically. From there you schedule a time directly with the clinician who reached out to you, typically within the same week for Modoc County patients. Your first appointment is a full intake session, 50 to 60 minutes, not a short screening call, and this is also when your Partnership HealthPlan coverage is confirmed. Sessions run on Doxy.me, a HIPAA-compliant video platform that requires no download and no account, and phone-only sessions are available if your internet is unreliable.

  • In-person appointments — PMHS is 100% telehealth by video or phone

If You Move Counties

If you move to a county Partnership HealthPlan does not cover, you will typically lose your PHP eligibility — this is what we have seen happen with our own patients. If that happens, we talk through your options with you directly. That usually means private pay with PMHS, or asking your Medi-Cal case manager about what is called an in-county transfer, which reassigns your coverage to a PHP county. An in-county transfer requires traveling to that county to see your primary care provider, since PHP coverage is tied to where your primary care is based. There is no need to sort this out alone — reach out and we will walk through what makes sense for your situation.

Getting to Know Modoc County

Modoc County is high desert and mountain range with almost nobody in it, and that openness is exactly what draws people who live here. Surprise Valley, the South Warner Wilderness, and the wildlife refuge outside Alturas are not weekend destinations for most residents, they are simply what is out the front door.

Is there really no local option in Modoc County?

Local licensed mental health providers are extremely scarce, which is exactly why PHP contracts with telehealth providers like PMHS. Your enrollment gives you access to care that does not depend on anyone opening an office in Alturas.

My cell service is better than my internet. Can I still do therapy?

Yes. Telephone sessions are a fully covered, complete form of care. A significant share of our most rural patients use phone for every session.

Will anyone in town know I'm in therapy?

No. There is no office, no waiting room, and no vehicle parked anywhere. Your care is between you and your clinician, protected by the same confidentiality laws as any medical treatment.
