Marin County
Marin County, just across the Golden Gate Bridge from San Francisco, has approximately 291,000 residents across 520 square miles (SimpleMapLab, n.d.) and is one of the more affluent counties in the state, with San Rafael as its county seat and largest city. That affluence is not evenly distributed: West Marin, the county's largest region by land area, is largely rural and agricultural, with small coastal communities like Bolinas, Point Reyes Station, and Stinson Beach that are a world away from the county's wealthier eastern cities.
PHP-enrolled Medi-Cal members in Marin, particularly in West Marin, can face real gaps in accessible, low-cost mental health care despite the county's overall wealth. PMHS provides telehealth therapy to PHP members throughout Marin County, from Sausalito to the far side of Point Reyes.
Coverage & Access
PHP covers outpatient therapy with no session cap for medically necessary care and no copay for most Medi-Cal members. Marin's own county health needs assessment has found a suicide rate well above the California average (Healthy Marin, n.d.; MarinHealth, 2019), along with higher-than-average use of medication for mental health conditions, a pattern county health officials tie in part to housing costs and to gaps for residents who fall between Medi-Cal eligibility and what they can afford out of pocket.
You do not need a referral, a member card, or prior authorization to reach out. Submit the intake form and we will contact you within 12 to 24 hours, whether you are in San Rafael or out past Point Reyes Station.
Language Support
PMHS arranges interpreter support for patients across Marin County, including Spanish and other languages. Note your language preference when you submit the intake form and we will have it ready before your first appointment.
Do I Qualify?
- Your Medi-Cal is enrolled through Partnership HealthPlan of California, based in Marin County
- You are physically located in California at the time of each session
- Any age qualifies — EPSDT covers medically necessary care for members under 21
- No referral or prior authorization is required to start
Why Telehealth Works Here
Marin County's wealth is the exact reason its Medi-Cal members struggle. In one of the most expensive counties in America, PHP members, concentrated in San Rafael's Canal neighborhood, Marin City, Novato, and West Marin, live and work inside an economy priced for someone else. Private-pay therapy in Marin is among the costliest anywhere, most local therapists do not take Medi-Cal at all, and the result is a county full of clinicians that its lowest-income residents cannot see. Telehealth through a contracted PHP provider bypasses that market entirely. A housekeeper in San Rafael and a caregiver in Point Reyes Station get the same access, with no bus transfer and no lost shift.
Care From PMHS
PMHS serves Marin PHP members with individual therapy for anxiety, depression, and trauma, support for service and care workers holding up the county's economy, interpreter support for the Canal's Spanish-speaking community, and adolescent care under EPSDT. Evening and weekend appointments fit multi-job schedules, sessions require no download or account, and phone sessions are available, including in West Marin where coverage thins. Covered members pay nothing, need no referral, and face no waitlist. We respond within 12 to 24 hours, with most consultations held the same week.
Both PMHS clinicians, Ryan Frost, LMFT, and Dmitri Kolpacoff, APCC, see patients throughout Marin County. There is no assignment by location — whichever clinician has availability is the one who reaches out to you.
Your First Session
After you reach out, your clinician contacts you directly, always starting with a text, then by phone or email depending on what you prefer, to confirm you are ready to get started. You will then get an email invitation from SimplePractice, our patient portal, to finish setting up your account and sign your consent forms electronically. From there you schedule a time directly with the clinician who reached out to you, typically within the same week for Marin County patients. Your first appointment is a full intake session, 50 to 60 minutes, not a short screening call, and this is also when your Partnership HealthPlan coverage is confirmed. Sessions run on Doxy.me, a HIPAA-compliant video platform that requires no download and no account, and phone-only sessions are available if your internet is unreliable.

If You Move Counties
If you move to a county Partnership HealthPlan does not cover, you will typically lose your PHP eligibility — this is what we have seen happen with our own patients. If that happens, we talk through your options with you directly. That usually means private pay with PMHS, or asking your Medi-Cal case manager about what is called an in-county transfer, which reassigns your coverage to a PHP county. An in-county transfer requires traveling to that county to see your primary care provider, since PHP coverage is tied to where your primary care is based. There is no need to sort this out alone — reach out and we will walk through what makes sense for your situation.
Where Is Marin County?
Approximate location. PMHS is 100% telehealth, so this map is here to help you get oriented, not to confirm eligibility — what qualifies you is your PHP enrollment, not your exact address.
Getting to Know Marin County
Marin County sits between the Pacific coast and San Francisco Bay, and the Marin Headlands and Mount Tamalpais are not scenic destinations people travel to visit. They are where Marin residents hike on weekdays and where trails are crowded on weekends because the county is surrounded by open space.
Marin County FAQ
Every therapist in Marin seems to be private pay. Am I actually covered somewhere?
Yes, here. PMHS is a contracted Partnership HealthPlan provider, which means your PHP Medi-Cal covers your sessions at no cost for most members, regardless of what the local private market charges.
¿Atienden a la comunidad del Canal en San Rafael?
Sí. Interpreter support is available, sessions happen from your home or any private place, and evening times work around long work days.
I feel strange struggling in a wealthy county. Is that something therapy addresses?
Yes, and it is more common than you think. Financial strain inside an affluent area creates its own isolation and shame, and it is a fully legitimate focus of covered care.
